It's not 'hard', but it does need a thicker oxide on the I/O - but very cheap (sub 50c) 8 bit Micros are available with 1.8V core and 5.5V io, and there are numerous 32 bit ones with 5V IO too.
Infineon have a lot of 5V automotive parts, 8/16/32 bits, and some new ones are even 6-27v, with included Regulators and LIN drivers.
